Sacramental Registration Forms

Baptism Registration Form

Church legislation requires that those who wish to receive the Sacrament of Baptism, in a particular parish must have a real connection with that parish – either by living within its boundaries or through registration, practice (regularly attend Sunday Mass) and supporting the good works of the parish. Note that registration at Our Lady of Sorrows School or past family history of sacraments received at Our Lady of Sorrows do NOT constitute parish registration.

Parents are required to attend a Baptism Preparation class (registration required) offered every third Tuesday of the month from 7-9pm in the parish hall.

Baptisms are usually held every first Sunday of the month at the 11am and 12:30pm Masses.

RCIA Registration Form

The Rite of Christian Initiation of Adults [RCIA] is the way adults come to learn about and experience the Christian life, specifically our Roman Catholic tradition, and become initiated into the life of our Church through the celebration of the Sacraments of Initiation: baptism, confirmation and eucharist.

Registration begins mid- September 2017.

First Reconciliation and First Communion

Church legislation requires that those who wish to receive the Sacrament of First Reconciliation and First Communion, in a particular parish must have a real connection with that parish – either by living within its boundaries or through registration, practice (regularly attend Sunday Mass) and supporting the good works of the parish. Note that registration at Our Lady of Sorrows School or past family history of sacraments received at Our Lady of Sorrows do NOT constitute parish registration.

Registration forms will be available mid-September 2017. Registration form, payment and other pertinent documents required need to be personally submitted to the parish office.

Confirmation

Church legislation requires that those who wish to receive the Sacrament of Confirmation in a particular parish must have a real connection with that parish – either by living within its boundaries or through registration, practice (regularly attend Sunday Mass) and supporting the good works of the parish. Note that registration at Our Lady of Sorrows School or past family history of sacraments received at Our Lady of Sorrows do NOT constitute parish registration.

Religious Education

The Religious Education program is available for children of registered parish families who do not attend Catholic schools. This 8 year program for students in grades 1 – 8 is mandatory for those children who wish to receive their sacraments at Our Lady of Sorrows, if they are not attending a Catholic school.

Registration is processed between the months of May to June 2017.

Teacher Reference

It is critical for you to know that to be hired to teach in any Catholic school board in Ontario, a letter of reference from your pastor is required as part of your application for employment.

Candidate for teaching positions are expected to be practicing Catholics who are faithful to the Sunday Eucharist. It is also highly recommended that you are registered at the parish and establish yourself as a volunteer within the parish community.

Mass Intentions

We have 2 types of Mass intentions available:

Announced - mentioned at the beginning of the Mass; published in the Sunday bulletin - $20
and
Unannounced - no specified date; unpublished - $10.

Please note that we do not offer Perpetual Masses.

Parish Registration Form
Registered members can request offertory envelopes from the parish office.
Pre-Authorized Giving (PAG)
The PAG Program allows parishioners to request that a monthly total be transferred from their bank account to OLS Parish on the 20th of each month, for their regular Sunday offerings.
PAG Pledge Form
Required for enrolment in PAG, this form allows parishioners to specify the details of their sacrificial giving.
